Requesting Medical Records:
To request a copy of a medical record, please use the following steps:
Call or Email our Medical Records Department. You may call or email the Medical Records Department at records@pavillon.org or 828-694-2300. You will need to complete a form for these records and the department members will help you determine what form you need and the quickest way to receive it.
There is no charge for records as long as they are sent electronically. This includes fax or email. If records are emailed to you, it will be sent via a link from a program called ShareFile. You will need to put in your name and email to download.
